The Adult Jobs Report (AFR) is a questionnaire that the Social Security Administration (SSA) uses to determine if you qualify for disability benefits. The Social Security Act (SSA) requires this form to be completed so it can be confirmed that your disability is severe enough to be eligible for a disability benefit.
You need to fill this in with information about your disability indicating that your symptoms will last for at least twelve months because this is the main requirement to assess your eligibility for a disability benefit.
What is an adult jobs report?
It is a 10-page form called Form SSA-3373 that the SSA uses to determine how your medical condition affects your daily life. Once you become disabled as a result of your medical condition, you must complete a questionnaire and provide honest and specific answers to the questions being answered.
You only have a limited time to complete the form, which is usually 10 days from the date of the letter with your Social Security application. You certainly shouldn’t be in a hurry when completing the form because it remains on your record for the entirety of your case and cannot be modified.
What questions are included in the Adult Jobs Report?
There are several questions you need to answer on the form which may include any of the following:
- Do you need help taking care of anyone else or pets?
- Do you spend time with others, such as your family members or friends?
- What are your hobbies and how often do you participate?
- Do you need help bathing, dressing or feeding yourself?
- Do you go to certain places regularly despite your disability?
- Do you cook your own food and how long does it take to complete this task?
- Are there things you used to do that your disability now prevents you from achieving?
The fifth question is one of the hardest to answer as it asks, “How do your illnesses, injuries, or conditions limit your ability to work?” To answer, you should not only list the symptoms of your medical condition but should describe exactly the tasks you could not perform in your previous jobs that forced you to stop working or have your employment terminated by your employer.
When is an adult job report filled out?
Once you submit your initial application, the Social Security Administration (SSA) will ask you to complete the form SSA-3373. This is usually a month after you have submitted your initial application.
If you are asked to complete an adult jobs report, the SSA tries to understand what activities you are still able to do despite your disability.
It will assess the Residual Functional Capacity (RFC) report that you would have completed in the presence of your physician or in the presence of a Social Security Administration (SSA)-appointed physician.
